I understand these plans have limitations on prescription coverage

I understand there is no network for hospital and facility coverage. Those services must be pre-authorized through the Care Coordination team

I understand working with the Care Coordination team is required and they can direct me to a different facility

I understand, and am willing to invest time with Clearwater to make these programs successful

I understand Trident Tier 1 benefits might not be available in all area's and is not guaranteed

I understand the Trident program is guaranteed issue without pre-existing limitations; however, current treatments and Rx might not be covered

I understand ClearShare is not insurance, and has pre-existing limitations with lifetime maximum sharable amounts

The STANDARD Program By TRIAD is currently available for 1099 individuals as well as Companies of 1 or more employees offering full network access through the CIGNA PPO as an alternative to the Clearwater Program.
